Could Metabolic Change Begin Before Polyp Formation?

Exploring a possible window between healthy colon-cell metabolism and visible disease

The conventional model of colorectal cancer development follows a progression from genetic and cellular changes to benign adenomas and, in some cases, malignant cancer. Screening is designed to interrupt this process by finding and removing precancerous polyps.

The EpiNutrition hypothesis asks whether another important stage may occur earlier:

Could colon cells begin to undergo abnormal metabolic changes before a visible polyp forms?

Researchers have detected Warburg-like metabolic and mitochondrial changes in normal-appearing colorectal tissue from people who had precancerous adenomas elsewhere in the colon. These findings suggest that the biological field surrounding a lesion may already be changing, even when the tissue appears normal.

This observation does not prove that metabolic reprogramming occurs before adenoma formation. It also does not establish that such changes can be reversed through nutrition. It does, however, raise an important possibility: colorectal carcinogenesis may involve an early metabolic phase that deserves further investigation.

The Proposed Sequence

The hypothesis explored in this series is:

Dysbiosis and altered microbial metabolism may reduce supportive signals such as butyrate, contributing to a Warburg-like shift in colon-cell metabolism. This altered metabolic state may affect gene regulation and cell behavior before benign polyp growth becomes detectable.

If this sequence is confirmed, the interval between metabolic disruption and visible tissue change could represent a previously underrecognized opportunity for prevention research.

This is a scientific hypothesis—not established clinical guidance. Recommended colorectal cancer screening remains essential.
